Is the displayed dB an exact measurement?

It is an estimate. The boost is shown in real decibels with WHO-based Safe, Caution and Danger zones, but the value is derived from your system volume and the applied gain, and real loudness varies by device, headphones and content. The app is not a medical device, so if you notice ear pain or ringing, lower the volume and consult a professional.

Can I use VolumeBooster+ on an Android car head unit?

Yes, if your head unit has Google Play and runs Android 7.0 or later. The setup screens are laid out for wide, short landscape displays. How much louder it gets depends on the head unit's own amplifier and speakers.
